All BC dialysis units have the same requirements and use the same processes and forms for visiting patients.
Visiting Patient Guideline
This guideline for Travelling/Visiting Hemodialysis Patients discusses the requirements and processes for accepting visiting patients at hemodialysis units in BC. Patients may come from within BC, within Canada or internationally.
Patient documentation must be provided in English.
Individual forms to be completed are provided below or, if preferred, all forms are contained in this file.
Visiting unit to send to home unit after patient contacts to request space:
Home unit to send to visiting unit (within 2 weeks of receiving Form 1a):
Home unit to send to visiting unit (immediately prior to the patient's visit):

Dialyzer Specifications and Substitution Chart for Visiting Patients
This chart provides a list of commonly used dialyzers and BC equivalent dialyzers. This may help reduce the need for patients having to bring their own dialyzer if the dialyzer they use is not in common use in BC.
Wall Poster
This poster is intended for posting in areas of high patient traffic. The purpose is to (1) raise awareness amongst patients that dialysis units support travel; and (2) that patients need to plan their travel ahead.
